I certify under penalty of perjury that I alone will study the Street Rules course materials and take the test without any outside assistance. I understand that if it is determined that I have not completed Street Rules without outside assistance, make false statements or present falsified documents to the court or Street Rules, that it may be a felony and the completion of this course will not be accepted and my fees will not be refunded. I have received permission from the court to enroll in this traffic school and understand that if I have received my citation from a court, but have not received permission from that court, and still continue through completion of this course, it will not be accepted and my fees will not be refunded.
